Product Permission & Caregiver Responsibility Form

I give my permission for my Girl Scout to participate in council-sponsored product programs for Girl Scouts of New Mexico Trails, Inc. (GSNMT). This includes the 2025 Fall Product Program and 2026 Girl Scout Cookie Program.

I Acknowledge

My Girl Scout and our family will follow GSNMT and GSUSA policies and procedures for council sponsored product program including the GSNMT Fall Product Program Family Guide and GSNMT Cookie Family Guide.

I Acknowledge

My Girl Scout and our family accepts responsibility to follow all troop deadlines and requests as set by the troop volunteer.

My Girl Scout and our family will review and follow the Girl Scout Internet Safety Pledge. We will also review and follow the online marketing guidelines for each product program as listed in the family guide.

I agree to accept financial responsibility for all products and money my Girl Scout receives. This includes recovering money from "bad" checks. I will only accept checks from customers I can contact if my troop allows them. I will also keep a copy of all receipts for product received and money turned in to troop volunteers. I understand I am responsible for any monies owed and all outstanding balances will be sent to a collection agency by GSNMT if unpaid. I also understand any outstanding debts will limit our participation in future product programs.

Our family understands that all monetary proceeds belong to the council and troops and they are not retained by individual Girl Scouts as their property.

I Acknowledge

Our family understands that all products are to be delivered promptly to customers including girl delivery orders. We will ensure our Girl Scout contacts customers in a timely manner with a delivery date. We understand that we are financially responsible for any undelivered girl delivery orders that result in refunds to customers and may have an outstanding balance after refunds are process. We will also refund customers for any cash payments we receive that we are unable to deliver.

Our family understands that our Girl Scout cannot sell prior to the program/sale start date, and we understand that our Girl Scout and their troop may be penalized for any early orders.

My Girl Scout and our family will to the best of our ability follow the Girl Scout Promise and Law during the product program.
